Website Privacy and Consumer Health Data Policy

This policy explains how Wellify Health handles information connected with your use of this public website. It also provides disclosures for consumer health data that may be subject to the Washington My Health My Data Act. Clinical records and other protected health information are governed by our Notice of Privacy Practices and applicable health privacy laws.

Information collected automatically
When you visit the website, our hosting, security, and website analytics providers may process technical information such as your IP address, browser and device type, pages requested, referring page, date and time, and diagnostic or security logs. This information is used to deliver the website, maintain security, prevent abuse, and understand basic website performance.
Information you choose to provide
This public website does not currently include an intake or contact form. If you call, email, use the secure patient portal, or request an appointment through a linked service, we receive the information you choose to provide through that channel. Do not send urgent or highly sensitive health information through ordinary email.
How information is used

- Comply with legal, regulatory, billing, and recordkeeping obligations.
- Protect patients, visitors, Wellify Health, and others from fraud, misuse, or safety threats.
Wellify Health does not sell personal information or consumer health data. The public website does not use third-party advertising trackers or personal information for targeted advertising. Google Analytics is configured with Google signals and advertising personalization disabled, and Wellify Health does not intentionally send contact, appointment, patient portal, or clinical information to Google Analytics.
Consumer health data disclosures
Visiting pages about mental health services can create website interaction data that may reveal or support an inference about a person's interest in health care. To the extent this information is consumer health data and is not exempt because it is protected health information or health care information governed by other law, the following disclosures apply.

Wellify Health has no affiliates that receive consumer health data through this public website.
Your privacy choices and requests
Depending on the law that applies, you may ask whether Wellify Health holds consumer health data about you, request access or deletion, withdraw consent for future collection or sharing, or ask for a list of third parties and affiliates that received the data. We may need to verify your identity before acting on a request. We will respond within the period required by applicable law.
If we decline a request, you may appeal by contacting us again and writing “Privacy Appeal” in the subject line or at the top of your letter. We will review the appeal and explain our decision. Washington residents may also contact the Washington State Attorney General.
Service providers and external links
We use service providers to support website hosting, security, analytics, communications, scheduling, and patient operations. They may process information only to provide their services to Wellify Health or as otherwise allowed by law and contract. Google Analytics helps us understand aggregate website use; Google processes analytics data under the Google Privacy Policy. The homepage also displays public rating information supplied by Google Maps Platform. That rating request is made by the website server and does not send Google the health information you may provide to Wellify Health. Links to IntakeQ, the secure patient portal, Psychology Today, Google Maps, or other external services take you to websites governed by their own privacy practices.
Retention and security
We retain information only as long as reasonably needed for the purposes described here, to provide care, or to meet legal and professional obligations. We use administrative, technical, and physical safeguards appropriate to the sensitivity of the information. No internet transmission or storage system can be guaranteed completely secure.
Adults only and policy changes
Wellify Health provides services to adults age 18 and older. This website is not directed to children. We may update this policy as our website or legal obligations change. The effective date above identifies the current version.
